LAWSUITS
Nathan Health Care Center, LLC, East St. Louis, is suing Cozette Carlisle, rural Alton, for $46,564.50, plus interest, attorney fees and court costs, that the plaintiff claims is owed for nursing home care provided for Carlisle’s late husband, John A. Carlisle. According to the suit, the nursing home provided care for John Carlisle after he was seriously injured as a result of a motorcycle accident. The suit alleges the defendant entered into an agreement to pay $3,000 a month via electronic transfer to satisfy the alleged debt. John Carlisle reportedly died Feb. 13. Nathan Health Care Center filed a claim in probate in excess of $56,000 in October against the Estate of John A. Carlisle.
William Smith, Gillespie, has filed a forcible entry and detainer complaint against David Rhodes II, Sawyerville, seeking $1,110 that Rhodes allegedly owes Smith for 2014 and 2015 taxes, plus possession of residential premises located in the 400 block of Pfahler Street in Sawyerville.
SMALL CLAIMS
Michael J. Hailstone, Gillespie, is suing J.B. Murr/Kirby Co., Peoria, for $1,200 Hailstone claims to have paid toward a Kirby vacuum cleaner that was never delivered.
SLUCare, St. Louis, is suing Thomas K. Komnick, Bunker Hill, for $766.98, plus court costs, that the plaintiff claims is owed for medical services.
Tower Loan of Illinois, Springfield, is suing Carol Smith, Carlinville, for $2,464.42 that the plaintiff claims is owed on an allegedly defaulted loan.
Discover Bank is suing William O. Mack, Girard, for $2,185.38, plus court costs, that the plaintiff claims is owed on an allegedly defaulted credit card account.
Discover Bank is suing Taylor Bartok, Mount Olive, for $2,771.46, plus court costs, that the plaintiff claims is owed on an allegedly defaulted credit card account.
In separate lawsuits, H & R Accounts, Inc., as assignee of Springfield Clinic, is suing several area residents for money the plaintiff claims is owed for medical services. The defendants and judgments sought are as follows: Marney Turner, Virden, $537.64, plus court costs; Debra Ochur, Girard, $1,559.34, plus court costs; Candice Price, Sawyerville, $1,045.70, plus court costs; Debra Smith, Gillespie, $864.95, plus court costs; and April and Gary Thompson, Mount Olive, $711, plus court costs. In separate suits, Midland Funding, LLC, is suing Janice Mills, Shipman, for $3,138.01, plus court costs, and $961.01, plus court costs, allegedly owed on two defaulted Citibank credit card accounts.
Midland Funding, LLC, is suing Renee Carlen, Staunton, for $858.39, plus court costs, allegedly owed on a defaulted Credit One Bank credit card account.
Lisa Higginbotham, Gillespie, is suing Meghan Waterman, Mount Olive, for $300 the plaintiff claims is owed for daycare services.
Midland Funding, LLC, is suing Jon Chester, Medora, for $1,611.79, plus court costs, allegedly owed on a defaulted credit card account.
Premier CDL Training Services, Granite City, is suing Brian S. Finke, Staunton, for $7,102.67, plus court costs, that the plaintiff claims is owed on an allegedly defaulted installment loan agreement executed for a CDL training program.
Capital One Bank is suing Stacey R. Piechocinski, Benld, for $1,855.73, plus court costs, allegedly owed on a defaulted credit card account.
St. Francis Hospital, Litchfield, is suing Michael Fenwick, Mount Olive, for $4,432, plus court costs, allegedly owed for medical services.
MTC-1, Hazelwood, Mo., is suing Jennifer M. Fultz-McConnell, Staunton, for $7,351.87, plus court costs and $350 in attorney fees, that the plaintiff claims is owed on an allegedly defaulted installment loan agreement executed for CDL training.
Patricia Struberg, Plainview, is suing Caleb Mabus, Plainview, for $2,000 allegedly owed on a car loan from Struberg to Mabus.
Midland Funding, LLC, is suing Patricia Ladd, Royal Lakes, for $2,113.38, plus court costs, that the plaintiff claims is owed on an allegedly defaulted Fingerhut credit card account.
Crescent Bank and Trust is suing Michael L. and Heather Barnhouse, Virden, for $5,433.12, plus court costs and $350 in attorney fees, that the plaintiffs claims is owed on a defaulted $34,312 auto loan executed in 2012.
Capital One Bank is suing James M. Conway, Virden, for $1,958.63, plus court costs, allegedly owed on a defaulted credit card account.
Cavalry SPV I, LLC, is suing Martin Custer, Shipman, for $9,509.86 allegedly owed on a defaulted Chase Bank credit card account.
Midland Funding, LLC, is suing Crystal Harvill, Wilsonville, for $1,489.67, plus court costs, allegedly owned on a defaulted Citibank credit card account.
FELONIES
Shawn M. Fugate, 28, Virden, is charged with escape for allegedly failing to report to Macoupin County Jail for incarceration on Nov. 27. Originally convicted of criminal sexual assault involving force, Fugate was sentenced to a year in jail in 2012 followed by six months of probation, according to court records. Earlier in November, Fugate’s probation was revoked and he was sentenced to serve 120 days of jail time.
Ryan Wheeler, 22, Chesterfield, is charged with driving on a revoked driver’s license, obstructing a peace officer and aggravated assault in connection with a Dec. 2 incident in Chesterfield. The driving on a revoked driver’s license charge was elevated to a felony due two previous convictions for the same offense on Wheeler’s record, both in Macoupin County. According to the complaint, Wheeler disobeyed a command from a Macoupin County Sheriff’s deputy to show his hands, fled back into the Sportsman Inn, slammed the rear door and locked it. Additionally, Wheeler is accused of pointing a BB gun that substantially resembled a handgun at another person driving past the scene of the incident on West Depot Street. In a separate filing, Wheeler is charged with criminal damage to state-supported property for allegedly damaging a light fixture belonging to Macoupin County about two hours after being taken into custody.
Bryer D. Greff, 21, Virden, is chard with possession of cannabis with intent to deliver in connection with a Feb. 25 incident during which he allegedly delivered 10 to 30 grams of a substance containing cannabis to an undercover police agent.
Nikolaus C. Stepniewski, 39, Springfield, is charged with driving on a suspended driver’s license in connection with an Aug. 27 incident in the northbound lanes of Interstate 55 at milepost 44. The charge was elevated to a felony due to three previous convictions for the same offense in Sangamon County.
Dena M. Allen, 43, Gillespie, is charged with unlawful delivery of methamphetamine in connection with an April 9 incident during which she allegedly delivered less than five grams of a substance containing methamphetamine to an undercover police agent.
Jonathan M. Robbs, 28, Virden, is charged with unlawful delivery of methamphetamine in connection with a July 22 incident during which he allegedly delivered less than five grams of a substance containing methamphetamine to an undercover police agent.
John C. Weffenstette, 57, Virden, is charged with unlawful delivery of methamphetamine in connection with an April 28 incident during which he allegedly delivered less than five grams of a substance containing methamphetamine to an undercover police agent.
Amanda S. Warren, 31, Gillespie, is charged with possession of a controlled substance in connection with a July 25 incident in Gillespie during with she allegedly possessed less than 15 grams of a substance containing heroin.
Stormy L. Ray, 29, Staunton, is charged with possession of methamphetamine in connection with a Dec. 11 incident in Staunton during which she allegedly possessed less than five grams of a substance containing methamphetamine.
Sandra M. Basket, 47, Virden, is charged with theft and unlawful possession of a debit card. Basket is accused of using a debit card belonging to Girl Scout Troop 6438, Virden, to gain unauthorized control over $500 to $10,000 in cash between April 6 and July 22.
Mark A. Elliott, 27, Virden, is charged with theft for allegedly taking $500 to $10,000 in cash from Circle K, Virden, between June 1 and Sept. 10.
MISDEMEANORS
Thomas D. Coyne, 23, Staunton, is charged with possession of cannabis in connection with a June 6 incident during which he allegedly possessed 2.5 to 10 grams of a substance containing cannabis.
Ricky M. Starr, 35, Wilsonville, is charged with theft in connection with a Sept. 26 incident during which he allegedly exerted unauthorized control over an LG gPad valued at less than $500.
Arnold J. Collins, 53, Asheboro, North Carolina, is charged with obstructing a peace officer in connection with a Dec. 11 incident in Girard during which he allegedly tossed away an item of drug paraphernalia as a Girard police officer was investigating an traffic accident in which Collins was involved.
Kelsey E. McKinney, 27, Carlinville, is charged with criminal trespass to property in connection with an Oct. 23 incident during which she allegedly entered onto Macoupin County Housing Authority property in Carlinville after having received notice from the Housing Authority that she was banned from the property.
Edward R. Wasylenko, 19, Gillespie, is charged with possession of cannabis and possession of drug paraphernalia in connection with a Sept. 5 incident in Benld during which he allegedly possessed less than 2.5 grams of a substance containing cannabis and a green pipe used for smoking marijuana.
TRAFFIC
Ronald J. Schuler, 22, Farmersville, is charged with two counts of driving under the influence of alcohol in connection with a Dec. 9 incident on East First South Street, Carlinville. Additionally, Schuler faces traffic charges for improper lane usage and failure to reduce the speed of the vehicle he was driving to avoid an accident.
Jill M. Smith, 41, Gillespie, is charged with driving under the influence of alcohol in connection with a Dec. 10 incident in Chesterfield. She also faces traffic charges for operation of an uninsured motor vehicle and failure to reduce the speed of the vehicle she was driving to avoid an accident.
Howard S. Ogden, 60, is charged with driving under the influence of alcohol in connection with a Dec. 4 incident on U.S. 66 at Old Route 66 in Mount Olive. Additionally, Ogden is charged with improper lane usage and illegal transportation of alcohol by a driver.
